What can I expect after I submit intake inquiry?

Cornerstone will process your intake request within two business days. If you are inquiring about health psychology services, Dr. Hendricks will be in touch via email to schedule a complimentary 15-minute discovery call to discuss referral concerns, review procedures/policies, and to schedule your initial appointment if appropriate.

If you have not received a response within two business days, please check your email settings.


Who can give permission for treatment?

For youth (anyone under 18-years of age), a legal guardian/parent is responsible for consenting to treatment. However, we feel it is critical to treatment that we obtain assent from children and teens and involve them in the process of treatment decision-making in a developmentally appropriate way.

In the case of divorce, it is our policy that both parents must provide consent for treatment, unless the presenting parent can provide documentation that they have sole legal custody or sole medical decision-making authority. Please notify us in advance of custody/legal issues.

For adults (anyone over 18-years of age), the person who is interested in services should reach out to our office themselves to inquire about possible treatment. You are welcome to give us consent/permission to communicate or coordinate with another person on your behalf, and we encourage that for dependent-adults. Otherwise, you will be responsible for scheduling, completing intake paperwork, and consenting to services.


How do I complete paperwork before my initial appointment?

Initial paperwork, including policies/procedures, consents, and preliminary questions, will be sent to you through the client portal once you are scheduled for your initial appointment. These forms will help us gather information that is essential to your care and help us to better understand your concerns and goals for treatment.

Because it is important that we have time to prepare before your first visit, we require that intake paperwork be completed at least 24-hours in advance of your appointment.


What can I expect during my initial appointment?

The first 1-3 sessions will comprise your initial consultation. One of our primary goals for the initial consultation is to get to know one another. During this phase before treatment starts, Cornerstone wants to ensure your comfort and to make sure that you are matched with the right providers to best meet your needs. Together you and your provider will discuss your concerns, your goals for treatment, and you will collaboratively come up with a plan for your care.


What can I expect during treatment?

Cornerstone’s health psychology sessions are typically 45-minutes and are generally scheduled weekly or every other week depending on your concerns and goals for treatment. Our approach to treatment emphasizes evidence-based techniques and regular measurement of progress toward your goals. Cornerstone care will always be compassionate, respectful, and collaborative.

Please know that your privacy is of the utmost importance to us. As such, we will keep everything we discuss confidential except in the following instances for which healthcare providers are required to release information outside of treatment: 1) imminent threat of harm to self or others, 2) suspected abuse of a child or dependent adult, and 3) court subpoena. We will discuss privacy and confidentiality during your first appointment.


How do I schedule or reschedule?

To cancel or reschedule your appointment, you may contact your provider by email or secure messaging through the client portal. We try to be as flexible as we can, but please note that our providers often have limited availability to accommodate or reschedule late notice requests.


How do I find out if I have out-of-network benefits?

We encourage you to contact your insurance company to determine if your plan has out-of-network behavioral health benefits. Depending on your plan, you may be eligible for reimbursement for a portion of your up front costs. Some families find that their insurance company will cover up to 50-75% of session costs, which ultimately may make your end costs similar to that of a co-pay for an in-network provider. 

When you contact your insurance company, you may wish to ask:

  • Do I have mental/behavioral health benefits?

  • Does my plan have out-of-network benefits?

  • How much does the company allow for the following codes:

90791—Diagnostic evaluation 

90834—Individual therapy, 45-min

90837—Individual therapy, 60-min

90846—Family therapy without patient

90847—Family therapy

  • How much does my plan cover/reimburse for an out-of-network behavioral health provider?

  • Is Authorization required? If so, how do I obtain authorization?

  • How/where do I submit out-of-network claims?

  • Do I have a deductible? If so, what is the amount and has it been met for this year?

  • When does my calendar year start?

  • Is there a limit on the number of sessions my plan covers per year?

  • Will my provider need to submit treatment plans?

What is a “Good Faith Estimate?”

You have the right to receive a “Good Faith Estimate” that explains how much your care at Cornerstone Health & Wellness will cost.

Under the law, health care providers need to give patients who do not have insurance or are not using insurance an estimate of anticipated costs for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items and services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least one (1) business day before your medical service. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.
